# See answers and scores

> Read a patient's answers and scores, and apply changes to their details.

The patient's **Forms** tab shows each form's status. A completed scored form shows its score, severity and any alerts.

Select **View results** on a completed form to see the scores, any alerts, and every answer.

To see forms across the practice, open **Forms › Sent**.

## Apply changes to patient details

Some forms offer to update the patient's details. The form then shows that updates are ready.

1. Select **View results**.
2. Check each change against what's on file.
3. Select **Accept & apply**.

Nothing changes on the record until you do this. Only admins and reception can apply changes.
