# Build a form

> Set up a form's pages and questions, link answers to the patient's details, preview and publish.

Only admins can build forms.

1. Open **Forms** and select **New form**.
2. Give it a name and category, then select **Create**. The builder opens.
3. Add pages and questions from the outline on the left. Select any row to edit it on the right.
4. Select **Preview** to see the form as the patient will.
5. Publish it and select **Save**.

To start from an existing form, select **Duplicate**.

## Questions

Select **Add question** and choose a type: choices and scales for questionnaires, text, date and address fields for patient details, and tick boxes, signatures and file uploads for consent. Turn on Required if the patient must answer.

For choice questions, you can pick a ready-made scale instead of typing every answer.

## Update the patient's details from a form

An intake form can offer to update the patient's record, such as their mobile or address. Open the question, open **Patient record**, and choose the field the answer goes to.

Nothing changes on the record until a staff member accepts it. See [Apply changes to the patient's details](https://docs.bilby.health/forms/answers-and-scores.md#apply-changes-to-patient-details).

## Save and publish

Changes aren't saved until you select **Save**. Bilby checks the form before it publishes and tells you anything to fix first.

A form you've already sent keeps the version it was sent with. Changes apply to forms you send from then on.
