# Forms

> Build forms, send them to patients and read the answers.

Forms are questionnaires patients fill in online: intake details, consent, and scored assessments such as the PHQ-9. You build them on the **Forms** page and send them from a patient's record. The answers come back to the patient's **Forms** tab.

## Who can use this

## The forms library

Open **Forms** in the sidebar. **Library** has the forms you can send, **Drafts** has forms not yet published, and **Sent** has every form sent to patients.

Every practice has common assessments ready to send, such as the PHQ-9, GAD-7, K10 and DASS-21, with their scoring set up. You can't edit these. Select **Duplicate** to make your own copy.

Only published forms can be sent. Use a form's **More actions** menu to publish, archive or delete it. A form that has been sent can't be deleted, so archive it instead. Forms already sent keep working.
