# PDF forms

> Send a PDF for the patient to fill in and sign.

> Not every practice has this yet. If you can't see it, contact Bilby support.

A PDF form sends your practice's own PDF. The patient downloads it, fills it in and sends it back. The completed copy lands in their **Files** tab. Only admins can add PDF forms.

1. Open **Forms** and select **Upload a PDF**.
2. Choose the PDF, give it a name and category, and select **Upload**.
3. Publish it, then send it like any other form.

If it's a published questionnaire, check your practice is licensed to use and share it.

To change the PDF later, open the form and select **Replace the PDF**. Forms sent and not yet returned get the new one.
